Hi Hichem, The twitching may be middle ear myoclonus. Check out tensor tympanic, stapedial, and palatal myoclonus. Middle ear muscle spasms that can be very disturbing. Often brought on from stress/anxiety or other neuro disorder. A good ENT will be able to confirm. Hope you feel better.

Palatal Myoclonus is notorious for not responding to one specific med. Multiple have to be tried. Unfortunately sometimes it is stuff like Klonopin that works. Do you know if it happens while you are sleeping?

I'm not sure, because this does not sound exactly like that, but middle ear myoclonus/TTTS present itself tipically in intermittent sounds, but more like thumping or clicking.

It could be some sort of middle ear myoclonus if you have hyperacusis.

I may be a bit late, but I recently read that stapedious myoclonus can cause clock-ticking like tinnitus. Maybe you want to read more about it?

Yes maybe I have stapedial myoclonus and not TTS then. But I think I have mild TTS also, it's the sound that resembles a flapping flag.

There seems to be some neural communication both afferent and efferent between the brain and the organs in the ear. I think this explains why people like me also suffer from myoclonus in the middle ear muscles.

Apparently there is a study showing that Levetiracetam has been used to treat people suffering from middle ear myoclonus. It makes sense to me that it eliminated my TTTS... this is my 3rd day or so with absolutely no spasms.

A very slight popping sound in my right ear. Consistent but intermittent. Can't find a trigger. It's really quiet. Like a light pop or a click but it doesn't seem like myoclonus. Almost like something is wet in there. Driving me nuts particularly at night.

Middle ear myoclonus is clicking tho isnt it? The videos I saw of that are not even close to low humming.
