Yes - 4 points.

Sometimes - 2 points.

Result:
Level 1. 0-16 points. Easy.
Level 2. 18-36 points. Moderate.
Level 3. 38-56 points. Average.
Level 4. 58-76 points. Heavy.
Level 5. 78-100 points. Catastrophic.
 
I took this test several months ago and scored in the 80s in the catastrophic range. Today, I scored in the 5os.

I'm going to take that as a positive sign. My tinnitus volume has not changed nor has my hearing recovered, but at least my anxiety and depression have improved.

I am very happy for you! That is great, and a significant improvement. I will retake this to use it as a measure of progress. I wonder if this survey could be pinned at the top of some section so that I does not get lost to time and is always in a prominent enough position that everyone has the opportunity to use this as a self assessment.

Does anyone know if this test is accurate enough to be used as a valid self assessment tool?

I think it is fairly accurate in terms of examining emotional distress as long as we are honest with our responses.

For example, I still have anxiety in many of the listed areas. But by taking the test again, I realized most aspects had moved from always to sometimes present.
 
12 I think the key issue with T is anxiety about it getting worse. If I knew that my T is not going to get any worse then what it is today I wouldn't think about it again.
 
This is not a competition :), and like I said earlier, scoring is like golf, lowest score wins! So far @Sam Bridge seems to be atop the leaderboard. (and sorry you scored so high)

NOTE: I know and care nothing about sports, any accuracy in my sports-related analogies is pure, dumb, luck.

I scored the lowest which means its easier to deal with..
 
30, I still feel like crap. I'm really at the point where it's the anxiety that's getting me down.

I find that having habituated and then spiked and habituated again and spiked again leaves me with dread that as I navigate my way to yet another habituation, which I feel is coming, I dread the day it punches me in the face again.

Not at all, I was hoping the smilie would indicate I was joking. Apparently I failed to get my point across. I have both read enough of your posts and interacted with you enough to not question your intelligence. I apologize if I accidentally offended you, that was not my intent.

I have profound deafness in one ear, constant T and burning sensation in the deaf ear, and reactive tinnitus to every sound I hear in my good ear. I had to quit my favorite hobby (playing music), retire from working, and reign in my social life. I score in the upper 50's. If I had taken this test during the early months after sudden hearing loss, I would have scored much higher. The functionality type questions have remained the same but I don't have the strong emotional responses anymore.
 
I scored 40. bear in mind when taking the test your mood at the time will probably affect your score so taking the test over several months and then averaging out the results will likely give a better overall picture.
 
What question did you answer "3" to?
I was catastrophic in the past at a audiologist and actually was trying to have a bit of dark humor as I didn't take the test. I have a severely loud buzz right now as I awakened. It's as loud as I can take. I apologize for my dark humor as it is real this morning. I feel for everyone here with this awful affliction. I don't know if it is ear, brain, or neck caused, but I have to do something to calm it down.
