Hair Cells/Nerve Damage Progression?

Would someone pls explain as to once the bacteria from otitis (ear infection) or loud noise for that matter damaged or destroyed some hair cells and /or nerve(s) in the cochlea (or nearby) why doctors are saying that it (hair cells/nerve damaging) may get worse down the road? What is there to damage it further?
