I've had tinnitus for about 10 years. I evaluate my symptoms on a scale from 1 (barely noticeable) to 5 (very annoying).
After reviewing many articles on B12 supplementation for tinnitus relief, as well as contributions from this forum, I found that the benefits of B12 appeared dubious at best, but not harmful. Despite this, I decided to experiment with a B12 supplement.
I chose Nature's Bounty Vitamin B-12 2500 mcg (300 quick-dissolve tablets) at less than 10 cents per tablet, available at Costco. Although labeled as quick-dissolve, the tablets take about three minutes to fully dissolve under the tongue. Research on B12 absorption indicates that sublingual absorption is not superior to oral absorption, but I proceeded undeterred.
I did not take the B12 supplement regularly. Instead, I used it only when my tinnitus was particularly annoying. I would dissolve one tablet under my tongue once or twice daily until I noticed a reduction in symptoms to a level 1 or 2.
Strictly from my personal experience over the past ten years, this has been the best 10 cents I've ever spent! Since starting supplemental B12, I have had many more good tinnitus days than bad.
