Cause of Tinnitus
Noise and/or infection
I have severe unilateral tinnitus from acoustic trauma, which started about 10 months back.

I often feel a dull ache in my ear canal, about an inch in. It usually corresponds with tinnitus spikes. It's unrelated to noise, so I don't think I have what folks call hyperacusis or noxacusis.

Does anyone else have anything like this?

Of course, my ENT was useless.

After experiencing acoustic trauma, acoustic shock, or barotrauma, significant changes can occur within the ear. You might feel pain in your middle ear even without being exposed to noise.
