My ear aches from time to time with the (suspected) Middle Ear Myoclonus that I have. Sometimes it feels like the ear canal, sometimes it feels like something past the eardrum (Eustachian Tube, Stapedius, etc. I'm not even sure).

Is there anything that this could point to?

As you are describing it, it could be anything. It could eustachian tube pressure, or some middle ear muscle playing up, plus some inflammation. I bet no doctor will be able to tell. Actually, the way middle ear muscles work is not fully understood.
 
There could be many things causing ear pain not just the various types of hyperacusis (cochlear noiception, central gain, lack of mylein around the AN nerve, CNS disorders)

Middle ear infections, eustachian tube disfunctions, inflamation, ear drum damage will also cause ear pain.


The word hyperacusis shouldn't even exist and every separate condition should be named after it's pathology.
