Cause of Tinnitus
most likely from acoustic shock/trauma
Hi everyone! :welcomesignanimation: I came across this forum from googling about tinnitus because it's been bothering me for over a month now (1 month and 2 weeks). I've been reading some success stories for while and a few not so successful stories which I try to avoid so I don't go down a negative spiral. I just want some positive insight from people who have come out of this successfully and how the process of fading went.

This might turn out to be a long post... so I will put it in paragraphs if you want to pick and choose what to read.

How it started
How I assume I got tinnitus, is from a loud car alarm that played IN a store so it was pretty fricken loud. It was on for about maybe 5-10 seconds. If I had known this would cause tinnitus I would have stepped outside. The next day I noticed a very high pitched EEEEEEE tone... which I normally hear when I'm in a very very very quiet enclosed area, which I learned recently to be tinnitus, but thought nothing of it back then because it didn't affect me until now. NOW I hear it over my tv, fan, and daily noises. The only time I don't hear is when I'm cooking or showering or out running errands. I can hear it faintly while I'm driving because I don't usually blast my radio. It's quite annoying and I can hear it at the market or store if I look for it.

How I felt/feel
In the beginning of the month I was really depressed because I couldn't focus on anything. I thought this would be the end of me... I couldn't enjoy my shows, or concentrate on homework... I was not feeling like myself and being a very negative person. Currently due to COVID-19 everything has been on hold. I used to love going out to the clubs and bars but after reading stories... it seems like everyone is saying to stay away from loud noises... I also work at a bar but it's not too loud and I may have to start working again once everything opens up. After maybe 3 weeks? I started to tell my self to just live with it... I've felt less depressed but I still feel very annoyed and down not knowing if I can go back to my normal life. I'm 29 and still living the party and social life so I don't know how I'm going to cope with not being able to go out to the clubs with friends.

What I have tried (doctors, treatments)
I called my GP and he prescribed me Prednisone because he thought my eardrums could be inflamed due to the acoustic trauma and referred me to an ENT. So I took a 8 day course starting with 40 mg reducing it down to 10 mg before stopping. I don't think it helped. I saw the ENT and he said my hearing is fine, nothing blocked, just basically my ears looked good... so he told me to buy Lipoflavonoid to see if it works... I took that for a month, I don't know if it was working... but I'm no longer taking it and it seems to not have gotten worse... I just started taking one a day vitamins since people have been saying to take supplements.

How my ears feel and how things have changed over the month
So I am 1 month and 2 weeks into this irritating noise. I don't think I have hyperacusis because noises don't hurt my ears... loud dishes or quick spurts of loud sounds don't seem to spike my tinnitus or if it does, very minimally? I do feel fullness in one ear or the other but it's random and doesn't last the whole day and appears maybe once or twice a week. In the beginning I had this weird pain in my ear, felt like a sharp pain that comes and goes, but now I rarely feel it. The ringing was unbearable in when I first developed it maybe like a 4/10 and this is weird but when I lie down it gets louder like 6/10 and it was a constant high pitched ring (eeeeeeee). So I decided to sit up majority of the time but all I wanted to do was lie down and relax... but I couldn't because the ringing would become louder. Then it started fluctuating... I would notice the pitch going higher but kinda twitching? I don't know how to explain... like it was broken or something... but when I lie down, it goes back to the steady EEEEEEE tone... at the end of the month I hear all sorts of tones... the annoying EEEE which is the loudest and the worst, the high pitched fluctuating tone, sometimes a tone that sounds like a high pitched vacuum, a high pitched buzzing sound and a high pitched hissing sound... and this is where I'm stuck at because I feel like it's going down but then the annoying EEEEEE comes back and I'm like... ughh noo not this again... but now it's occurring less than in the beginning of the month. The best it gets is to a hissing sound that sounds like air is coming out of something... but when I lie down it gets to louder hissing/buzzing sound? So I really can't tell if am improving or not due to all these fluctuations. I'm currently at a buzzing/hissing noise on a good day... which I have been having. The buzzing is still annoying but better than the high pitched EEEEE but I really just want silence... the occasional annoying high pitched EEEE likes to pop up here and there and it gets me depressed again because it's like a step back... now I say it's at a 3/10 unless I lie down it will go up to a 5/10. I don't know if I'm improving or just staying the same because I'm getting used to it (I can still hear it over stuff) but I really want to be one of those success stories where it just goes away (not habituation)... I want to be happy again :(

Questions I have
For those of you who have recovered from your tinnitus, how long did it take? When did you see improvements? And how was the fading process? Like did it suddenly go away? Or did it just decrease until it was gone?

Are you back to your old life doing things you normally do?

No negativity please :(

I really don't want to wear ear plugs... I tried and my own voice is really loud when I talk... if I have to I will wear them to loud bars and clubs but not out to the store or normal activities.

THANK YOU ALL WHO TOOK THE TIME TO READ THIS. :thankyousign:

Hello @n_li808!

Looks like we are pretty much in the same boat.
Three weeks ago I've developed exactly the same symptoms you have reported after abusing in-earphones' volume for a few hours.

I'm experiencing the same issue when laying down to go to sleep. I could take your description of the different kind of noises that keep changing and use it to describe to my condition to the doctor.

For the sake of your health, use earplugs around noisy places to protect your ears. There some pretty good musician earplugs for sale that you can use for months and they are pretty much transparent, hopefully you will be cured before needing another set. I don't use them all the time because if I wear them for a few hours it seems like my tinnitus gets worse, maybe because I get more sensitive to noises when I finally remove them.

A few weeks ago I didn't knew the first thing about tinnitus and I'm far from being specialist, but I think if the noises keep changing at least something is going on. I'd be more upset if things remained the same.

Last Thursday I've started taking magnesium tabs and, coincidence or not, the ringing went down slightly 3-4 later. There's no evidence that magnesium reliefs tinnitus, but I thought it was worth a try.

There are some reports here of successful cases that took 2/3 months. We are within schedule, so keep a positive attitude.

BTW, how old are you?

@HZO That was a good read! I hope one day we both get rid of our tinnitus.

I'm 29.

I bought earplugs on Amazon, I believe the brand is "eardial" not sure how good they really are but I just keep them on my keychain.

As for being positive it's a struggle... some days are good and some day are bad... it even fluctuates in the same day and I don't know what to think of it. I do take supplements too, I'm taking the one a day multivitamin, hopefully it will help somehow.

Hey @n_li808,

That kind of earplugs was the one I was talking about. You should be ok with those.
I'm a few months older than you, actually I just turned 30 a few days ago. I'm facing tinnitus like it was a warning (not that I needed one) for myself that I'm no longer a teenager and I need to start taking some extra care about health related stuff.

Today was one of those fluctuation days for me. Started bad in the morning and during the afternoon it settled down a good deal. To the point that it was just a little ringing in the distance even with earplugs on.

We will be ok. Why wouldn't we? We are still young and compared to several cases reported our onset incidents are a "piece of cake". Try to keep focused on something else and think positive :)

@HZO,

I noticed yesterday and today I'm having the high pitch tone come back but in slightly lower volume. I prefer the soft hiss. It's so frustrating that it fluctuates so much. I have both hissing and ringing fluctuating in one day.

And yes I do have hope it will get better. Just needs time. I'm such an impatient person. I expect things to resolve itself overnight

So I'm about 6 weeks in from my tinnitus onset... and I feel like it was getting better because the high pitch eeeeeeee was fading away leaving me with low buzzing and hissing... but for some reason last night it came back full blast! The EEEEEE was there and it was loud I had anxiety and couldn't sleep... It was like how it was when I first got it, maybe even louder... I eventually fell asleep but I woke up this morning and it remains the same...

What happened?? Why did it spike like this? Will it go down again?? I hung out with a few friends 2 days ago but I didn't feel like it was too loud.

Same here @n_li808! But in my case the spike was caused by myself, I thought listening to podcasts through headphones at low volume was a good idea :banghead:

Can't you relate your latest spike to anything else other than hanging out with your friends? Was there music being played there?
I hope your ringing levels come back to normal soon. Mine already dropped a little bit after Monday's evening spike.

@HZO hey! My friends are kinda loud... but is that normal? Like a delay spike, I saw them on Sunday afternoon but it spiked Monday night... I did have coffee Monday morning I'm not sure if that had effect too...

I'm glad yours came down!

Mine is very slowly coming down again but ever since it turned into a hiss (about a week ago) I feel like any noise will cause it to spike to a buzz/ring... I just want the hiss 24/7 but if I go out for a drive and come home it will be a different noise... I don't know what to think of it. I'm back to being all depressed and down... I start work again Friday and I work at a restaurant... ugh I'm afraid.

In my case the noise induced spikes started a few minutes after the exposure.
Caffeine and alcohol are known to make tinnitus worse, so maybe that was it. Or maybe it just spiked because this thing has a life of its own.

Mine keeps changing all the time too. Just keep taking care and it will be ok. Consider using the earplugs at the restaurant if the noise is uncomfortable. I guess you should hear enough to do your job with them on.

@HZO you're right, this thing does have a mind of its own. I just never know what noise it wants to be next. I hope it was just from the caffeine... because I can cut that out no problem. I live in a noisy area. It's hard to avoid noises all the time. I just recently found out that coming out from a shower changes my tinnitus sound as well.

Has yours been back to pre spike yet?

Hi,

I'm sorry you are going through a bout with tinnitus, I know how distressful that is.
I can offer you some hope, as I have had loud piercing tinnitus 2 times in my life. The first time was 14 years ago from very loud machinery for about an hour. The tinnitus was horrible piercing loud, but it slowly faded to zero in about 18 months. My second tinnitus was from an outdoor concert 4 years ago, and it has faded almost to zero, but it took 4 years. (I think it took 4 years the second time because my ears were compromised the first time, so are "weaker")

IMO, you will make a full recovery, but it may take several more months. Most acoustic trauma tinnitus fades away in 6-18 months, but severe cases can take years to fade, and very severe cases my be permanent.
My advice is to take it easy on your ears for a while, no concerts, loud music or earbuds. I expect within 6 months this will have faded away.

@jjflyman Aww thank you for your positive feedback, I appreciate it! It gives me a little more hope...

While you were recovering, did you have setbacks as well? I get random spikes and I'm afraid the spike will stay that way.

I get random spikes and I'm afraid the spike will stay that way.
That is very common with tinnitus. I have had tinnitus for over 30 years (am 32 and always had it) and I still get random fluctuations.

What matters is not your hour-to-hour trend, but your month-to-month trend. Try not to worry if you get a bad day. Is it lower in general than it was 4 weeks ago? That's what matters.

@hans799 I feel like I do see Improvement because some days it is lower... but I can't call it improvement if it spikes back to how it originally sounded... I feel like its going a few steps back when it happens.

Hi @n_li808 ,

First of all, welcome to the forum!

I have similar experiences. I have fluctuating tinnitus on a day to day basis. One day it can be a 8/10 (loud EEE, swooshing plus ear pain) and some other days it can a 1/10 (very light hissing that music can mask) , there are of course days where it's somewhere in between. Mine used to fluctuating in a somewhat predictable manner. It's like I can always get 2 days of 1/10 in a row and I know for a fact that on the third day, it will definitely go back to 8/10 the next day... but this predictability is gone now. Does yours behave the same way?

The good news for me is, the general volume of the Tinnitus even on the bad day decreased. Although it was improving very slowly, it is definitely improving. Also, I can get more than 2 days of 1/10, but at the same time it can be just one day, not like the always 2 day thing in the past. But I will take 4 days in a roll occasionally over the always 2 good days "cycle".

For me, significant changes seem to happen right at the 1.5 year mark. After that, I notice little improvements here and there. Although not completely recovered, I hope to one day get to a point where it doesn't bother me anymore so that I can habituate.

From reading your experience, it sounded like you are already improving in just one month. There is a good chance that yours will fade away. For now, I would strongly suggest you to stay away from loud environments like movie theaters and loud restaurants where you can't even hear the other person in the same table speaking to you. Do not use earbuds or headphones for the time being. Your ears are hurt so give them space and time to heal. I am not saying you can't do those things I listed above forever, just stay away from them in the time being while your ears are trying to heal.

check out my story here if you have time, I documented my full experience in this post:

Hi @winstona, thank you for your insight! I don't notice any patterns for good or bad days... I would know how my day would go when I wake up cause it would either be high pitch or hissing... It would stay in that range for the day, although I have little spikes here and there throughout the day.

I noticed in your intro that you go to movies/bars with out ear plugs... does that cause any spikes for you or make it worse?

Also when did you start hearing the hissing??

I hope mines fades for good :( I'm having a hard time focusing on my school work and daily task... I just want to roll in a ball and stay in bed...

For me I always have my earplugs with me whenever I go out. I did have a few times when not wearing earplugs that it caused some sort of spike right after the noisy activity (like after a movie). The spike usually goes away in a day.

Now that I think about it, there was one time after a movie where the spike lasted more than a week. It scared me a bit but I survived it luckily after it went back to baseline after around 2 weeks.

It really is a judgement call for me on whether to wear earplugs or not. But I tried not to nowadays whenever I can avoid it because not doing so gives me the feeling that I am finally getting back to normal.

@winstona yea that's what I'm having a hard time coping with other than the nonstop ringing, I feel like I have to be so cautious now and I really just want my life to be how it was but I can't :/
I'm hoping for a miracle.

I would still be careful especially this early on to be on the safe side, your ears are hurt and they do need time to heal.

Also when did you start hearing the hissing??
Oh, the hissing started almost immediately after onset. I think what happens is when the ringing is bad, all different kinds of tinnitus fire at the same time. When it improves, the EEE sound disappears and only the hissing and the twitching is left.

@winstona yea I'll be careful... I work at a restaurant so hopefully it won't affect it too much.

That's good then, right? If it started after onset? My hissing didn't start till maybe about a month in. Also today is one of those bad days... (EEEEEE) it was a steady hiss yesterday... this sucks. Just when you think you're progressing it punches you in the face.

I have learned to manage my expectations properly with this condition after 3 years. Many times when you think things are improving, the next day this beast comes right back and hammers you to the ground. I think it is more realistic to do month to month comparison than day to day comparison...

@winstona how much have you improved over the 3 years?

The kind of ups and downs remains the same but I would say the highest volume of my tinnitus during onset was around 8-9 out of 10. Now it's a 6-7 at most. Also, I get roughly 10% good days in any given cycle in the past and now I get 40-70% good days now in a given cycle.

That is very common with tinnitus. I have had tinnitus for over 30 years (am 32 and always had it) and I still get random fluctuations.

What antibiotic eye drops did you end up using after your surgery? Any side effects?

So I'm about 6 weeks in from my tinnitus onset.
Sad to say that I have entered to 6th week. There is not much improvement in my tinnitus. Good thing is now it has become more constant and less spike-y in the middle of night. My ears are now not too sensitive to loud noise. I still remember the first 3 weeks, how my tinnitus used spike according to the external loud noise.

@Alfred87 Mine still spike... sadly... I get a lot of fluctuations of different noises as well. Was yours noise induced as well?
