Hey everyone,

I've had tinnitus for about eight years due to an acoustic trauma. Along with that, I've experienced intermittent dizziness, headaches, and eye pain.

For the past year or so, these symptoms have become a daily occurrence and don't seem to be improving. Has anyone else experienced something like this? Has anything helped to improve it?

I've undergone hearing tests, caloric testing, and VNG testing, and as far as I know, everything has come back normal.

This sounds like vestibular/ocular migraine. I have had the same issue since an acoustic trauma in February 2023, as well as floaters and light sensitivity in my vision. Nortriptyline and Gabapentin are helping, as well as magnesium, riboflavin, CoQ10, and vitamin D.

Thanks. I have tried Nortriptyline with a bit of success but I will try a few other things. The straight up eye pain is the weirdest symptom.
 
Do you have any neck pain or issues? I struggled with the same thing, and it turned out my traps were weak. I started going to the gym to work on it.

When I feel it coming on, I usually take some anti-inflammatory medication.

I don't have neck pain, but according to information I found online, it seems like it could be vestibular migraines. However, the symptoms can feel really strange at times.

I experience daily headaches and dizziness, and my tinnitus tends to worsen during an episode.
 
@nmackintosh, what @AverageJoe12 is following is Dr. Hamid Djalilian's migraine protocol. It involves a stepwise approach that incorporates several supplements and medications. Trying each one individually is often not effective.

I am thinking about starting the migraine program as well. Three months in, how has your loudness hyperacusis and reactive tinnitus improved? Are you able to do anything now that you could not do three months ago?

I've switched to Clomipramine in an effort to better manage my hyperacusis. I did feel that Nortriptyline, at doses up to 75 mg, was somewhat effective for me. While using it, I found myself less bothered by noise in my office.

In addition to hyperacusis, I deal with significant neck and shoulder tension, which is also a migraine symptom. For that, I'm continuing to use Magnesium and Gabapentin as part of my treatment plan.

I'll keep you updated on how clomipramine works for me. It's a tricyclic antidepressant, similar to Nortriptyline. One well-regarded researcher even believes it might be the most beneficial option for migraines:
