This is the nightmare I inflicted on myself at just 31 years of age. I cannot believe how stupid I was with headphones. Ugh I just want to off myself.

Don't blame yourself at all.
I don't know you in real life but you seem such a lovely warm person.
Hearing aids with maskers can help you.
Make every day count and live life to the full and don't let tinnitus take away your smile ...

I don't mean to interfere in your medical diagnosis, but you have a fairly large air bone gap in your left ear, larger as frequencies get low.
This is often an indication of a middle ear condition.
Don't get me wrong: you also have sensorineural hearing loss (like me, you have both), but the conductive part of your loss could be caused by something like otosclerosis. I'm unsure your conductive losses are to be attributed to your loud noise exposure.

Hasn't any ENT doc mentioned this to you? Have you done any fork tests? (Weber, Rinne) any CT Scan to check your middle ear?
 
You mean you've had an ear infection for 7 months straight???
When did you start treating it?
Been treating it for the last few days with antibiotics. Ive told the ENT numerous times about it through the months. Thing is, I had tubes put in in October of last year and as you can see from the audiogram, it made the right ear better but worsened the left. I told him about it and he said he didn't know what it was. About a month later, it started leaking fluid and I get vertigo whenever I put my ear plug in that ear to shower. I told him about that as well and he didn't do anything. Well it kept up and he prescribed ear drops that worked a little bit but it came back.

My hearing chart is much worst, and i have full blown high frequency tinnitus in both ears. You have to find positivity, it's hard but there are aspects of life that are still positive and enjoyable. Don't throw in the towel, you still have a bright future :)
 
I'm not a doctor, but that doesn't look like something that you could have inflicted on yourself. ENTs are not going to be able to help you, because your issues are too complex. You really need to see an Otologist before this gets even worse.

My hearing is really poor too. It sucks, but life can still be enjoyable.
