Is a Car or Train Quieter? My Son Can't Fly Due to Eustachian Tube Dysfunction

I have to take my son cross country to get to John Hopkins, where they have a Eustachian Tube clinic. There is nothing like that here in Texas. My son can't fly due to the disorder - his inner ear might get damaged due to the pressure changes. It's either drive, take a train or the bus. It's about a 23 hour drive, and a 48 hour train ride.
Which would be quieter, car or train?

I'm pretty sure that bus would be the worst of both worlds. It's been a long time since I've been on a train, though.
 
There are a couple of variables that affect the answer to this question @teacher34.

Most importantly: the trainline/train model vs the car model.

I'm not a car owner, or a US citizen, so this information won't help me help you, but it's an important factor in weighing up the better option, and including these details will likely be useful to any individual(s) who has/have experience of driving and/or riding US rail networks.

Also, yes, definitely avoid the bus; don't need to live in the US to tell you, they're just universally uncomfortable, dirty and loud.

Amtrak platforms are extremely noisy (I would say on same level as aircraft noise), but sitting on the train itself isn't too bad if you sit far from the doors. I have worn earmuffs when on the platform. Riding Amtrak is definitely much more noisy than riding in a car overall.
 
I would drive. Just because you have control of the entire trip. He can tell you how his ear feels, and if he needs to stop for a break then you can do so as often as needed. Take your time. You both will be more comfortable in your own car going at your own pace. Stay in a hotel a couple of nights so he can rest his ear. You can also map your route ahead of time and try to avoid hills as much as possible. Basically, you have more choices than being stuck on a train.

I would also take the car, but would also wear foam earplugs and Sony NC headphones during the trip.
