Pain Hyperacusis But No Muscle Spasms?

It seems most stories of people with pain hyperacusis also report TTTS like muscle spasms. I don't really have that.

Does anyone else have pain hyperacusis but no noticeable muscle spasms?

I have the exact opposite. I have loudness/annoyance/fear hyperacusis without any ear pain. However, I do experience tightness all over my head when I talk or am exposed to loud sounds. I also get the fullness in the ears.
