Acoustic Trauma from Microsuction
I would like to raise peoples' awareness about the dangers of microsuction, and that this is not an entirely safe method of wax removal.

In April 2016 I had wax removed in both my ears by microsuction. During the procedure there was extended periods of very high pitched whistling, which I now know to be called "clarinetting". It happens when the suction tool is blocked by either wax or dry skin. I didn't know that this sound should not have been happening, I wasn't informed about any dangers. This caused a not negligible permanent threshold shift in my hearing over several frequencies. I now have hearing loss (essentialy high frequency) high pitched and very loud tinnitus, ear pain and hyperacusis. My life has been ruined by this procedure.

I was not made aware of the dangers of this procedure by the ENT who carried out the microsuction procedure.

This is apparently rare (although I have read other reports on the internet when people state they have hearing loss due to microsuction), but it DOES HAPPEN...It happened to me.

PLEASE BE AWARE THAT MICROSUCTION CARRIES RISKS, I WAS NOT INFORMED AND HAVE SUFFERED IRREVESIBLE DAMAGE DUE TO THIS PROCEDURE.

@GregCA, unfortunately no, I don't have any statistics, I'd love to find some. I have read on other forums though people stating problems due to microsuction. One person states that they had a 30db loss following the procedure. I know my loss was drastic in the days following the suctioning, resulting in awful tinnitus and hyperacusis. This is all permanent. I'm trying to find out statistics myself to back up my case, the ENT is denying this can happen.

you can sue them

Why is this even happening - getting hearing loss during an ENT visit !!

Microsuction sucks !

I've had the same procedure done without any problems after. Took 5 min for both ears and i did a high pitch test a week after with normal functioning ears.

I'm sorry this happend to you but i do think it's as you say, a rare instance. Hope things get better as time goes.

I have it done every 4 monthes and so far havent had a problem. My ears develope wax quickly and hear much better when they are clean and seems to lower my T for I can hear more natural sound around me.

Make sure you use olive oil drops in your ears 2-3 times a day for 7-10 days prior to wax removal to soften up any crusted wax on and around your ear drum.

Your situation was unfortunate @Blue28 However, I believe microsuction is the safest method of removing ear wax as I've had the procedure three times. If olive oil isn't applied to the ear 2 to 3 times per day for 7 to 10 days prior to microscution, problems might arise. As in any medical procedure, one is also reliant upon the expertise, skill and professionalism of the health professional. I asked my ENT doctor, who is an Audiovestibular physician, which is the safest method of earwax removal, and she said mircrosuction.

I wrote the post below a short while ago about microsuction that some people might like to read.

Microsuction and Irrigation.

The ear is a delicate and sophisticated organ that some people might not give much thought to, as the majority of the time it's able to take care of itself, until something goes wrong with it. Attached to the side of our head we see the outer ear known as the Pinnia. It collects sounds from the outside world and funnels them down the narrow passageway inside our head that we know as the ear canal. The sounds are directed onto the eardrum which is situated in the middle ear.

The ear canal is lined with wax the medical term cerumen. Small glands in the ears produces the wax and protects the skin of the canal by preventing bacteria, dirt and other foreign substances from entering. Under normal circumstances the wax passes out harmlessly but sometimes a build up can make the ears feel blocked and one might experience hearing loss, itchiness, sometimes pain and even tinnitus. If using eardrops doesn't clear the blockage a visit to the GP surgery for removal of the wax using irrigation might be necessary or in some cases going to an ENT clinic for microscution.

A few people have contacted me and raised concerns about irrigation and microsuction and asked whether they are safe for use on people that have tinnitus? I also had a lengthy discussion with someone in this forum last week, who's adamant the tinnitus was made worse after having microsuction. This person had done all the right things. Applying olive oil eardrops to each ear for two weeks prior to the procedure and still ended up with problems. I therefore believe, the health professional doing the microsuction, wasn't as skilled and adept as he or she should have been. This is unfortunate, as the person in question had no way of knowing this and is now enduring more discomfort.

The old fashioned method of ear syringing, using a metal canister filled with water and squirted into the ear under pressure, to remove earwax is no longer used. This method was rather crude and abrasive and could cause problems to the ear. Modern ear irrigation is considered gentler and a safer method. I have had this done three times at my GP surgery without problems. Prior to it being carried out I used eardrops 3 times a day for 10 days to soften the wax.

When the earwax is impacted irrigation might be unsuccessful even after using eardrops. The next step is microsuction. As the name suggests, the wax is literally sucked out using a small handheld wand with a suction tip and is considered to be the safest method of wax removal. I have also had this done three times at my hospital ENT department, which was carried out my consultant who I had every confidence in and had no problems. She is a firm believer in the use of microsuction especially for someone with tinnitus. Again, eardrops should be used for 7 to 10 days before having it done.

A word of caution. Type into any search engine and you'll probably find a mirosuction clinic in your area, advising how safe and quick it is and that there's no need to use eardrops to soften the wax. Others might say just use it for a couple of days or the night before coming into clinic. I strongly advise you not follow this advice. People have contacted me after having microsuction and didn't use eardrops for 7 to 10 days and ended up with tinnitus that they didn't have before. When they returned to the clinic to complain they were quickly shown the door.

Every medical procedure carries a risk. It is just not ideal to go around with blocked up ears, hearing loss or experiencing increased tinnitus as a result of impacted wax. If one is paying privately, try to find a reputable clinic and use olive oil eardrops in the manner I've stated before having microsuction or irrigation. I also advise to use eardrops two or three times a week to keep the earwax soft so it can pass out through the ears naturally and help prevent build up.

Hi, I got my T after a microsuction too. I had my hear blocked with wax and I went to a doctor in a private hospital. He made me a microsuction straight away and I got T since then. It was in 2015. They don't believe me. And another doctor from the same hospital told me that I have to leave with it.. I was 35... nobody cares and nobody believes that it was from the microsuction...

Even if it's rare the microsuction caused the tinnitus, it still is her experience and true. And what I think about statictics: fck them.
There are so many cases which are not mentioned/published. How many have suicide tinnitus and have never posted. Or dont even know the existence of this forum

I had been using drops for approx 10 days prior to my ENT visit, the wax was soft so this wasn't the issue.

I have to stress the fact that I USED ear drops twice a day for approx 10 days before I had the microsuction procedure. I remember the ENT who did the microsuction saying that the wax was soft. So, in my experience, using drops prior to having wax extracted by suction, did not make for less complications. I STILL GOT TINNITUS, HEARING LOSS AND HYPERACUSIS, even after using drops to soften the wax before suctioning.

@Vera Lisa I'm sorry to hear you also experienced this, it's absurd that such so called treatments can mess up our lives in this way. Like you, people don't believe me. I created this post to try to get people to listen, and to raise awareness of the dangers of microsuction. It's unacceptable that patients are kept in the dark about the possible side effects of this treatment, it is NOT without risk. The consequences are VERY REAL, and life changing. WE have experienced this first hand. I wish you well.

Hi blue28..
I hope you get better too..
At the beginning I was very angry but with time I calmed down and accepted what happened to me. It make me feel better. I am trying to be more positive.
I do things that make me feel good and try to be happy. My T get worse if I stress.
I'm doing acupuncture but I have no results yet. But it makes me feel relaxed. Hope one day there will be a cure and we all be free of T... In the meantime we have to stay strong and do the best we can with our lives.
Take care.. (sorry about my English..I am Portuguese..)

Thank you for your kind message Vera Lisa, I'm glad you are feeling more positive. I'm still at the angry stage, it's hard to accept and let go. I hope, in time, I will also be able to move forward in a more positive way.

I'm trying to keep this post current as I can not stress enough the (not mentioned in my case) RISKS that CAN arise from having microsuction. Many people have had this treatment without problems, but MANY, like myself, have suffered hearing loss/tinnitus/hyperacusis or had existing tinnitus made worse by this procedure.

Allowing an ENT to use microsuction to remove wax was the WORST DECISION I ever made.

I had 18 shots in my ears for the AM101 trials (3 shots per ear for 3 rounds). The only time I had an increase in my tinnitus was after they suctioned one of my ears. That was only once out of the 18 shots and they only suctioned for a second or two. Immediately following the suction I had a very loud new tone that lasted about an hour, then faded into the background. After a couple of weeks the tone went away completely.

Lesson learned. I will never let them suction my ears again. I don't even mind getting IT shots into my ear as long as they use phenol and don't suction it out.

I know I am not the only person who has had tinnitus/hyperacusis/hearing loss brought on or worsened by ear wax removal (in particular microsuction). I am hoping to get in touch with as many people as possible who have had ear problems brought on or worsened by ear wax removal. If you are one of the unfortunate people that this has happened to then please be kind enough to leave a short message below. Thank you.

I had my ears done not by microsuction. My ears were very sore and when I went to see my doctor he said I see nothing in your ears they look very clean. So why the plugged feeling I asked. Gave me meds to help. It didn't work. So I made an appointment to have ear candling done. The amount of wax that came out was a lot. Two days later my T went silent. I have now a very light ringing in my right ear which I can live with. Microsuction I wouldn't do it

Microsuction I wouldn't do it

When earwax is properly loosened with olive oil eardrops used 3 x a day for 10 days prior to microsuction or irrigation, the procedure is usually quite safe, providing the health professional is competent. There is an element of risk in every medical procedure. It is just not ideal to walk around with blocked up ears caused by build up of wax. It needs to be removed. I have had microsuction 3 times and irrigation the same and had no problems. Please see my post above in this thread.

@Michael Leigh you have been very fortunate in your experience with microsuction. I had it done ONCE in my life and it has caused T, H and hearing loss...my life has been ruined by this procedure. As I said before, I used drops for 10 days prior to having wax removed, and the wax was very soft when it came out. The noise created during the procedure was NOT soft however, it was extremely loud and squealy...I believe this is what caused the problems. I was not informed prior to the procedure that this noise was not normal, so didn't know it was damaging. I now know that it's called "clarinetting".

Your experience with microsuction was an unfortunate one @Blue28 there is no doubt about that. As I have said, there is an element of risk in every medical procedure. If earwax won't come out of the ear canal by itself when applying eardrops there are only two choices one has. Either leave it alone and don't do anything. This can cause the following: Hearing loss, Earache, Tinnitus, Itchiness,Vertigo, Ear infections. Or get it removed. There are two procedures that are recommended to remove the eawax: irrigation or microsuction.

Hi
I read your post about microsuction and was very interested. I am looking at this stuff on behalf of my 15 yr old daughter who unfortunately had microsuction at the ENT department of our local hospital.
Immediately following this procedure she has suffered with extreme hyperacusis- bilateral ear infections which eventually settled with antibiotics, and she has had bouts of nausea - dizziness and ear pain. We returned to the hospital only to be told it's nothing to do with the microsuction and it's not even an ear problem. My daughter has missed a lot of school because she can't control the volume of the world around her, and I desperately want to help my child but no one will help us because it's not an ear problem!! Do you have any thoughts??

If your post/question is directed at me I can only advise you to see an ENT doctor or another type of Dr that hopefully can help your daughter. I am sorry to hear of her discomfort but cannot offer anymore suggestions. I have had microsuction 3 times and had no problems. Olive oil eardrops must be used 3 times a day for 7 to 10 days before having microsuction or ear irrigation.

@Vera Lisa - I had this too and it worsened. My Audiologist didn't soften the wax beforehand. He just went for it. Did yours? X
