    The big question is, when people experience a Tinnitus "spike" which they attribute to medication not listed as ototoxic, is that medication causing damage to the cochlear or is the medication affecting neurology causing the tinnitus to be sensed.

    It's an interesting theory, it explains why TRT works, could explain why some drugs are ototoxic and other drugs can increase tinnitus yet not be ototoxic. But I thought the prevailing view here is that tinnitus is caused by hearing loss?

    It's not "clinical" hearing loss because it's at or below 20dB. I suspect that's why your ENT said "good thing, we can rule out sudden hearing loss".

    Interesting, but did you have a spike in Tinnitus before you read online about Viagra? It's sad to read of your relationship troubles. I think TRT might be something worth investigating, where they play white noise just below the tinnitus, this might relieve your anxiety.

    Does anyone take Sildenafil? Does it effect your tinnitus or do you find it is okay? There aren't any good alternatives to PDE5 inhibitors, and Sildenafil has one of the shorter half lives.

    I should add to my comment that whilst I wrote "idiopathic", this should never discount that hearing protection should be used, and avoid headphones, loud concerts...etc when suffering from tinnitus.
