The thingy is to help you insert the plugs into your ears. the outer end of the plug fits in the thingy. Then you pull up your ear a bit with one hand, and with the other you insert the plug with the thingy. It's easy, you'll get used to it in no time.

You need to insert them pretty good with the thingy that comes with it. It gets softer after a while. You'll probably get used to them in a while. You can find different sizes and brands, but it's soft so you're probably just not used to it. About the breathing, you're overthinking here. if you...
