I've been doing some digging on surgeries for hyperacusis, specifically noxacusis. And I came across a research paper that said a musician with severe hyperacusis underwent a labyrinthectomy of their cochlear, and their symptoms were completely eradicated. The case is here but it's unfortunately behind a paywall:

Putting aside the fact that most doctors wouldn't do this surgery, why or why not do you think that this would work for pain hyperacusis?

This is only if you were positive that the pain was coming from the type II nerves and that setbacks occurred in the cochlear.
